Salary:

• Band 7, £43,742 to £50,056 per annum, pro rata

Job Summary:

The Medical People is seeking an experienced pharmacist for our client. This role involves working across different settings, including admissions wards, primary care, and care homes, focusing on medicines optimization for frailty, polypharmacy, and high-risk residents.

Key Responsibilities:

• Provide a medicines optimization service to care home residents.
• Deliver clinical services including patient drug histories, prescribing rationalization, treatment protocols, therapeutic drug monitoring, and compliance monitoring.
• Support in maintaining and improving pharmaceutical services within clinical management.
• Contribute to drug budget management through expenditure analysis and audits.
• Provide education and training to healthcare staff.
• Engage in audit, research, and development activities.
• Prescribe and review medications for therapeutic effectiveness.
• Liaise with healthcare professionals in primary care, with a focus on care across the interface and within care homes.
• Identify at-risk patients within care homes to minimize hospital admissions.
