About the Role:
We are recruiting for a Clinical Dietitian to join a hospital in Carmarthen, providing specialized dietetic services within the acute/urgent response team and integrated community teams. You will lead on training, education, and development of support workers and community teams, embedding best practices to proactively manage malnutrition risk and optimize patient care.

Key Responsibilities:

• Manage a varied and complex caseload of adults with nutritional support needs.
• Provide specialized dietetic advice and interventions in various settings, including patient homes, care homes, and through virtual platforms.
• Act as a dietetic lead within the community team, collaborating with nurses, therapists, pharmacists, and support workers.
• Ensure timely identification and management of malnutrition, providing training to multi-professional teams.
• Contribute to the development and implementation of nutrition and dietetics policies and standards.

Requirements:

• Expertise in adult dietetics with a focus on nutritional support and malnutrition.
• Strong clinical judgment and ability to manage complex patient cases.
• Experience working within multidisciplinary teams in community and acute settings.
• Ability to lead, train, and educate teams on best practices in nutritional care.
