About the role:

We are looking for someone who can lead, mentor and coach a team who provide high quality person-centred care and support in a supported living setting for individuals with complex care needs, including autism and behaviours of concern. The successful candidate will be responsible for the day to day running of Moorview Care’s supported living services, ensuring the team deliver safe, compliant and outstanding care and support.

Day to day duties:

  • Oversee the team, provide support, guidance and coaching to the team delivering care
  • Ensure the team provide support in line with client's individual support plans
  • Undertake and submit weekly reviews of care, apply manager commentary and submit reports
  • Support the creation and implementation of transitions, participate in assessments, attend discharge meetings and CPA/MDT reviews
  • Complete monthly audits on client finances and expenditure
  • Complete quality assurance checks of accurate record keeping, monitoring charts and welfare charts including BMC’s.
  • Undertake meds competencies, error investigations and audits for medication records. Schedule client medication reviews
  • Actively participate, create an agenda, and take notes for team meetings. Follow up on actions. Attend Operational Workshops & Ops Management Meeting when required
  • Conduct supervisions, ad-hoc documented discussions, annual performance reviews (APR's), personal development plans (PDP's), return to Work Interviews (RTWI) and any other 1-2-1 discussions with the team. Investigate grievance and disciplinary where required
  • Be able to drop in to perform all duties of a Support Worker and deputise in the absence of Service/Locality Manager

Eligibility Criteria:

  • NVQ Level 3 qualification or above.
  • Willingness to work flexibly and to keep knowledge and skills up to date.
  • Previous exposure to complex clients.
  • Enhanced Disclosure from the Disclosure and Barring Service.
  • Desirable - Full driver's licence with no more than 6 points and Class 1 business insurance (expenses paid for insurance where necessary).
