Key Responsibilities
Provide respectful high-quality support and care to Adults with mental health issues as per their individual support plans.
Provide assistance with activities of daily living.
Ensure that service users are encouraged to have companionship and social, emotional and leisure support in accordance with their support plan
Ensure that service users receive planned or urgent health care whenever required.
As key worker for individual service users, ensure that people’s assessed needs as stated in the support plan are comprehensively met.
Participate in the development of support plan in collaboration with service users, colleagues and other professionals.
Record essential information such as daily reports, care and health monitoring charts, financial transactions, accident and incident reports and telephone messages clearly, accurately and immediately.
